You’ll be asked for your personal information including social security number.
Fill out the short form and submit.
This completes a 5 year interim Opt-Out request which is immediately submitted to the 3 credit bureaus and becomes effective within 5 days of your request.
To make this request Permanent, print out the page and mail it in to the Opt-Out Department. All directions are on the site are very easy to follow.
